  Program Description
  A Health Home is a care management model for Medicaid recipients whereby all of the professionals involved in a member's care communicate with one another so that all medical, behavioral health and social service needs are handled in a coordinated, comprehensive manner.
This is done primarily through a care manager who oversees and coordinates access to the services a member requires while working to be sure that the member receives everything necessary to stay healthy.
Health Homes are designed to help people manage chronic health conditions to improve health outcomes and reduce the use of inappropriate inpatient and emergency room care.


  Admission Criteria
  Medicaid beneficiaries who have at least two chronic conditions including asthma, diabetes, heart disease, obesity, mental condition, and substance abuse disorder; HIV/AIDS, or one serious and persistent mental health condition.


  Treatment Services
  The care manager will work with the member to coordinate and facilitate access to an array of primary and acute physical health services, behavioral health care, and long-term community-based services and supports. This might be access to affordable housing, health coaching, education on disease management, and links to transportation for medical appointments to make sure people get regular treatment.
