Long Term Home Health Care Program
If you live in Oneida County and qualify for nursing home placement, perhaps you could be maintained in your own home with some extra support. The Long Term Home Health Care Program(LTHHCP) is designed to offer this support. The Oneida County Office for the Aging/Office of Continuing Care and the Visiting Nurse Association work in partnership to provide a coordinated plan of care and home care service through LTHHCP.
• The decision for LTHHCP is made by you, your family, your physician and
the professional health care team.
• Home care may be appropriate if you have relatives or friends willing and
able to help and a home environment that is safe and secure.
• The cost of LTHHCP may not exceed 75% of the average monthly cost of a
skilled or intermediate care facility in Oneida County.

To Qualify
- Be eligible for nursing home placement
- Desire to remain at home
- You must have your physician’s approval
- Have an assessment that determines the individual’s needs can be safely met at home
- Require case management and coordinated services
- Require a waivered service
- Remain within a budget for Medicaid provided services
- If you are Medicaid eligible, spousal or traditional, you must have approval for the LTHHCP from the local Department of Social Services
